Thank you for choosing us as your periodontal office. The following is our financial policy. Our main concern is that you receive the proper optimal treatment needed to restore your dental health. Therefore, if you have any questions or concerns about our payment policies, please do not hesitate to ask our office staff.
Payment is due at the time services are rendered. We accept cash, checks and credit cards. If you have dental insurance, then only the portion of the account that insurance will not cover is due at the time of service. We can be reimbursed for the remaining balance directly from your insurance company.
We will be happy to complete the treatment portion of the insurance form and make all necessary inquiries regarding your coverage in order to help you receive the maximum insurance benefits for care provided; however, we cannot be held responsible for any information from the insurance company. Also, please remember that all charges are your responsibility. Our relationship is with you, not with your insurance company. For that reason, any claims that have not been paid by the insurance company for 90 days after being submitted, will become the full responsibility of the patient and will be due at that time.
If payment cannot be made in full, we will be happy to structure an interest free payment schedule over a three month period. Any balance left after the three month period, or if you choose a payment schedule longer than three months, will be subject to an interest charge of 1 ½ % each month. In any case we cannot structure a payment schedule for longer than six months.
In the event of default, you agree to pay legal interest on the indebtedness, together with such collection costs and reasonable attorney fees as may be required to effect collection of this loan.
For our implant patients, we simply request payment of half of the fee at the time of implant placement, with the remaining balance due at the time of implant uncovering.
We understand that temporary financial problems may effect timely payment of your balance. We encourage you to communicate any such problems to the Office Manager so they can assist you in the management of your account.
